Pre-Admission

Planning ahead ensures a smooth and stress-free hospital experience. If your visit is not an emergency, take the time to prepare by following these steps:

1.

Bring valid IDs with personal details (e.g., current address, next of kin, employer).

2.

Keep a list of your medical history, including medications and records, ready.

3.

Bring your healthcare cards or insurance coverage forms, as well as any special form(s) or pre-authorization documents such as Letter of Guarantee (LOG) or Letter of Authority (LOA).

4.

If you are married, bring information about your spouse's insurance coverage to ensure that you avail yourself of full insurance benefits as his/her dependent.

Health Insurance

At Estevez Hospital, we understand that health insurance can be complicated. That’s why we have a dedicated PHIC Officer to help patients navigate their health insurance coverage. We accept PhilHealth and are contracted with several Health Maintenance Organizations (HMOs) and managed care plans.

If you are a PhilHealth member, please submit the following documents to our PHIC Officer within 24 hours of admission:

  • PhilHealth CF1
  • Copy of Updated Member Data Record (MDR)
  • PhilHealth Official Receipts
  • For admissions with procedures – 9/12 (9mos prior to confinement)
  • For admissions without procedures – 3/6 (3mos prior to confinement)
  • Valid ID of Representative (Front and back copy of ID)
  • Senior Citizen ID (Front and back copy of ID)

Health Insurance

For PhilHealth Members

Please submit the necessary requirements and forms to avail of PhilHealth deductions from your final bill. Our PhilHealth Dept. Officer will guide you through the process and explain the details thoroughly. To ensure your hospital bill is adjusted promptly, please submit the completed and properly filled-out PhilHealth eligibility and claim documents “before your discharge.” To avoid delays, you or a family member may begin working on these documents immediately upon admission.

For Private/ Government Members:

  • Member Data Record (MDR) - indicating the current name of the employer

For Indigent/Sponsored Member:

  • Member Data Record (MDR) — with indicated Validity Period for the present year

For OFW Members:

OFW members are divided into (2) subcategories:

Land based

  • Philhealth Receipts (at least Januany of present year — present month of confinement)

Seabased
  • Member Data Record (MDR): with indicated current name of Employer
  • Certificate of Philhealth Contributions from Employer
  • If on vacation, Philhealth Receipts shall be submitted. Contributions shall cover the succeeding months they are off-duty.

For Individually Paying Members:

  • Philhealth Receipts (for the present year)

Note: however if newly registered under this category the member is required to pay at least three months of the monthly contribution in order to be eligible for philhealth benefits use.

For Lifetime/Senior Citizen/PWD Members:

  • Member Data Record (MDR)

NOTE: In addition member/patients noted that were verified through the PhilHealth benefit eligibility form which prompted “YES” are advised to sign PhilHealth forms before discharge.

Visiting Policies

Visitors play a vital role in healing, and we encourage family and friends to bring positivity and support. To ensure patient comfort and visitor safety, we kindly ask everyone to follow these guidelines:

  • One watcher is advised to take care of the patient occupying our rooms, this is to prevent the spread of infections and to minimize unnecessary disturbance to other patients.
  • For children's safety and well-being, it is not recommended that children, ten (10) years old and below, visit relatives and friends confined in the hospital
  • Hospital staff reserves the right to limit visitation, as needed, to meet the healthcare needs of the patients or to protect the patient's privacy needs
  • If a patient expressly wishes or if the physician orders that visitors are excluded for medical/therapeutic reason, a "NO VISITORS" sign is posted on the patient's door
  • "No Smoking" policy is strictly practiced within the hospital buildings and premises to protect the health of our patients, visitors, and staff

Accepted HMO Providers

We accept a wide range of health insurance plans and are contracted with several HMOs and managed care plans. Access to our hospital requires an authorization from your insurance company or managed care plan. However, please note that this list is subject to change, and coverage limitations may depend on individual group contracts and company participation.
